HOW TO RECIEVE PTA SERVICES
Referrals are from a qualified healthcare practitioner. Speech Therapy needs a doctor’s referral ONLY when billing insurance or Medicaid patients.
The following form has been posted on our website to better serve you and facilitate your initial visit with us. Please print, complete, and bring form with you to your first visit.
It is our intention at PTA to offer the best therapy care to our patients and families. We endeavor to work with our patients so they receive the care they need without creating a financial burden.
Checks, Cash, and VISA/MasterCard Debit or Credit and Discover and American Express Credit.
Please provide us with the most up-to-date insurance information you have available. If you have a co-pay due with office visits, the co-pay is due at the time of your session. All charges will be filed with your insurance carrier. We will take assignment on your claim, which means the insurance company will pay the benefits due directly to either:
PT: Pamela A McDowell, PC.
OT: Waynette Jarrell OTR PC.
ST: Sandra A Mader, PC.
Any remainder due on your account will be billed to you in the form of a monthly statement.
Below is a list of our primary insurance plans. We are participating providers with most insurance plans. A few plans require prior authorization before an evaluation. Please contact your insurance carrier to verify your benefits and determine if the referring physician needs to obtain an authorization prior to the first visit. Please be aware that some insurance plans exclude therapy services.

All services are provided by appointment only.
Monday – Thursday: 8:00 am – 5:30 pm
Friday: 8:00 am – 12 pm
Kingsville (Physical Therapy ONLY)
Mon., Wed., Thurs., Fri. : 9:00 am – 5:00 pm
Scheduling time exceptions are made by individual therapists.
